NHS Solutions has a new opportunity in California for an Interim Clinical Social Worker. The successful candidate will be responsible for the day to day operations of facility wide utilization, discharge planning and care coordination. Our client is looking for a hands-on leader.

Key Requirements:
  • 5 years of clinical nursing experience in a directly related setting (acute care, skilled nursing, etc)
  • 3 years of case management experience
  • 2 years of supervisory or lead experience
  • Licensed in Clinical Social Work in California or California RN License
  • Active BLS – Basic Life Support Certification issued by the American Heart Association
  • CPI -Crisis Prevention Intervention Training
Key Skills:
  • Proficient computer skills including Microsoft Office suite
  • Excellent critical-thinking and analytical skills and professional judgment to assist staff in working with others in overcoming barriers to goal achievement
  • Adapt to metric-driven performance, supporting multiple stakeholders and achieving goals
  • Strong managerial skills including coaching and mentoring, problem solving, conflict resolution and motivational skills
  • Ability to create or drive new process, implementation, training, support and evaluation
  • Customer service focus and collaboration with stakeholders
  • Expert understanding of care management, regulatory requirements, third party payer criteria, critical timelines and level of service criteria

NHS Solutions seeks an experienced Interim Leader in Social Services for our client in California. Under general direction, the Manager of Social Services has direct responsibility for the daily oversight of the operations of an assigned area of responsibility in the Social Service Department, to include management of staff/resources such as the ability to hire, develop, motivate, train and discipline staff. Provides intensive case services to selected cases. Performs related duties as required. Key Requirements:

  • Master’s degree in Social Welfare from an accredited college or university or accredited Licensed Clinical Social Worker (LCSW) preferred.
  • 5 years as a Medical Social Worker; and 1 year of which must have been in a lead/supervisory role
  • Pro-active, strong problem-solving skills
  • Well versed in CMS, Title 22 rehab, and SNF regulations
  • Skilled in working with and teaching others in helping patients & family adjust to new health conditions and decision-making
