The successful candidate will be responsible for day-to-day leadership activities managing a team of clinical professionals in the execution of exemplary Hospital clinical social work and cross continuum care coordination. This individual works with the director, market leaders, physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care and services, and to execute collaborative practices that exemplify the Health System’s commitment to patient centered care and community engagement.
