NHS Solutions has a great opportunity for an Interim Director of Care Coordination in the New England area. Reporting to the CNO, the Interim Director Care Coordination has accountability and responsibility for planning, organizing, developing, designing, directing, and evaluating the resources to support patient care and transitions. The successful candidate will lead the clinical, financial, and operational improvement activities across the continuum of care, facilitating integration of services, communicating plans, developing, and implementing standards and programs while working collaboratively with other clinical leaders and senior administration to ensure organizational and departmental success. They are responsible for the supervision of Case Managers, Utilization Review RNs, and Social Workers, and will work to mentor staff and increase employee engagement. Will work with hospital leadership and community resources to ensure optimal transitions of care across the continuum.
